起動時に静かに失敗するハーネス自身の設定問題を調べる `config_doctor` ツールを登録します：設定全体の置換でパッチのフィールドが落ちた、存在しないエントリ id を対象にしたパッチ、ツール名の衝突。読み取り専用です。

起動時のガード：破損したセッションログを修復し、プラグイン構成を事前検査し、クライアント成果物を vm チェックし、host の apply() をスモークテストし、クラッシュの原因となるバンドルを隔離することで、壊れたプラグインが起動を妨げないようにします。
