Annotates bash results whose exit code 1 is an answer rather than an error (grep with no match, diff with differences, false test) so the agent does not re-investigate them.

Windowsのワークスペースを開く操作を修正。提供元の起動処理は`explorer.exe "<file:// URI>"`を実行しますが、一部ホストではウィンドウを開かず成功と報告するため、終了コードでは起動の有無を判別できません。この操作だけを`explorer.exe /e,<dir>`へ振り替えて確実にウィンドウを開き、メニュー内の他アプリは従来の方法のままです。
